Joe Manganiello Says Illness Strained His Marriage to Sofía Vergara

A private health crisis is now reshaping the public story of their divorce.

Joe Manganiello has opened a deeply personal chapter of his marriage to Sofía Vergara, alleging that their relationship became emotionally distant while he was battling a severe and prolonged illness. In his new memoir Bloodlines, the actor says he spent seven years dealing with intense abdominal pain, fevers, skin problems, fatigue and repeated medical procedures, including emergency surgery. Manganiello describes the experience as life-threatening and says it fundamentally changed both his physical condition and his emotional world. His account now introduces a new version of what was happening behind one of Hollywood’s most closely watched marriages.

According to Manganiello, the illness began not long after the couple married in 2015 and became progressively more serious. He lost significant weight, underwent multiple colonoscopies and eventually required surgery that temporarily left him dependent on an ostomy bag. Doctors treated him for several possible gastrointestinal and autoimmune conditions, but he says no definitive diagnosis fully explained what was happening. The prolonged uncertainty also forced him to use strong pain medication despite a long-standing commitment to sobriety.

Manganiello alleges that the strain extended directly into his marriage. He says the relationship had become emotionally empty and claims Vergara refused physical affection during some of his most difficult periods. He also recounts feeling mocked in connection with Bubbles, the small dog he adopted while struggling with his health. These are Manganiello’s allegations, and Vergara has not publicly confirmed his version of those private interactions.

The memoir also revisits the couple’s disagreement over children. Vergara previously said that differences over parenthood contributed to the end of the marriage, particularly because Manganiello wanted children while she did not want to become an older mother. Manganiello now offers a different account, saying that failed fertility treatments and his deteriorating health ultimately led him to abandon the idea of fatherhood because he feared dying and leaving Vergara alone with a child. The contrast illustrates how the same marriage can produce sharply different memories of why it ended.

Manganiello says his health later improved following additional surgery and that he is now symptom-free and no longer taking medication. He is also engaged to Caitlin O’Connor and says the possibility of becoming a father has reopened. His memoir appears designed partly to reclaim control over a period of his life that had previously been interpreted mainly through public statements surrounding the divorce.

The larger story is not simply about celebrity conflict. It is about how illness can alter identity, intimacy, dependency and memory inside a relationship, especially when both partners are living under constant public scrutiny.

Private pain often becomes public only after the relationship is over.
