New imaging research suggests that social inequality can become biologically embedded.

Living in a socioeconomically disadvantaged neighbourhood may be associated with poorer structural brain health, according to a study examining 2,826 clinical magnetic resonance imaging scans. Researchers found measurable differences involving brain age, total tissue volume and signs of vascular damage.
The study compared participants’ residential areas using the Area Deprivation Index, which incorporates indicators such as income, employment, education and housing conditions. People living in the most disadvantaged areas had brains that appeared approximately two years older than their chronological age on imaging-based estimates.
Researchers also observed lower total brain tissue volume and more white-matter hyperintensities among residents of deprived neighbourhoods. These abnormalities are commonly associated with damage to small blood vessels and have been linked to cognitive decline, stroke and dementia risk.
The findings do not demonstrate that neighbourhood disadvantage directly causes brain deterioration. The research was cross-sectional, meaning that scans and socioeconomic conditions were examined at one point in time. Individual medical histories, duration of residence and other unmeasured influences could partly explain the associations.
Several pathways nevertheless offer plausible explanations. Chronic stress, air pollution, limited access to nutritious food, unsafe environments, inadequate healthcare and fewer opportunities for physical activity can accumulate over many years. Their combined effect may increase inflammation, cardiovascular strain and vascular injury affecting the brain.
The study reinforces a broader public-health principle: neurological health is shaped not only by genetics and personal behaviour, but also by the environments in which people live. Preventing cognitive decline may therefore require investment in housing, healthcare, green spaces, education and neighbourhood safety alongside conventional clinical interventions.
