AI Reveals Migraine as More Than a Headache Disorder

Biological patterns suggest that one diagnosis may conceal several distinct conditions.

Norwegian researchers have used machine learning to identify people with migraine without providing the system with information about their headache symptoms. The study, published in the journal Neurology, analysed health and genetic data from more than 43,000 participants in the Trøndelag Health Study, including almost 9,000 people classified as having migraine.

The models examined demographic characteristics, mental health, cardiovascular and musculoskeletal conditions, sleep, exercise, medication use and genetic information. The strongest model achieved an area-under-the-curve score of 0.80, indicating reasonably strong discrimination between migraine patients and people without headaches. Genetic data improved its performance only marginally beyond the clinical information already available.

Age emerged as the most influential predictor, followed by factors including neck pain, menstruation and nausea. According to the researchers, the model’s ability to detect migraine without examining the defining headache itself suggests that the neurological disorder leaves a broader biological and clinical footprint throughout the body.

A second analysis examined 12,185 participants with sufficiently complete headache information. The algorithm identified a group of 1,425 people in which 94% met the researchers’ migraine criteria and divided it into four subgroups. One consisted entirely of men, another showed prominent neck pain, a third combined musculoskeletal pain with anxiety and depression, and a fourth resembled classic migraine with neurological aura. Differences in genetic signals between the groups supported the possibility that migraine may represent a spectrum of related biological conditions rather than one uniform disease.

The findings could eventually help explain why a treatment can work well for one patient but provide little benefit to another. They may also contribute to more consistent diagnoses and individually selected therapies. However, the model is a research result, not an approved diagnostic test, and migraine continues to be identified primarily through clinical evaluation because no definitive blood test, scan or biomarker currently exists. Independent validation in newer and more diverse populations will be necessary before the approach can guide routine medical decisions.
